General
The United States of America have dominated the women's 4x100m medley relay event since it was introduced at the 1960 Games. They have won 10 out of 15 Olympic gold medals on offer to date, including at the two most recent editions in 2012 and 2016, and the last time the Games were held in Tokyo in 1964.

The US team took home silver on the four occasions they failed to win Olympic gold, including in 2004 and 2008 when Australia won. They did not take part in the 1980 Games in Moscow.

At the world championships the US team have claimed seven world titles, which is ahead of the People's Republic of China and the German Democratic Republic, who both have four, and Australia on three. Like the Olympics, the US team claimed gold at the two most recent editions of the tournament in 2017 and 2019, and set a world record in Gwangju with a time of 3:50.40
