The ROC qualified for the women's 4x100m medley relay in Tokyo via the 2019 World Championships in Gwangju. Previously, the Russian Federation has competed in the event at all six Olympic Games from 1996 onwards, and have made the final at four of those Games. Their best result remains a fourth-place finish in London in 2012, where they set a new national record but missed out on a medal by 0.30s to Japan. At the 2016 Games in Rio de Janeiro they swam even faster than they did in London to set another national record, but once again it was not enough to make the podium.

The team has fared better at the world championships, picking up two bronze medals in 1994 and 2013, as well as a silver in 2017 in what remains their fastest time [3:53.38] in the event at an Olympic, world or European championship event. They could not keep that momentum going at the most recent edition of the world championships, however, as they failed to even qualify for the final at the 2019 edition in Gwangju.

At continental level, the Russian team were unable to defend their gold medal from the 2018 European Championships at the most recent edition of the tournament in 2021, where they finished second behind Great Britain by more than two seconds.
