Germany won a silver medal at the 1992 Olympic Games in Barcelona, where they competed as a reunified nation for the first time. They won a second medal, a bronze, in another outdoor pool at the 2004 Games in Athens but that remains the last time they made an Olympic final in the event. The Germans narrowly missed qualification at both the 2008 and 2012 Games, recording the ninth-best qualifying time both times before dipping to 12th at the 2016 Games in Rio de Janeiro.

Prior to reunification, the German Democratic Republic, the Federal Republic of Germany, and the Unified Team of Germany all won Olympic medals in the event, but only the German Democratic Republic claimed gold [1976, 1980, 1988].

At the world championships, Germany have claimed three medals since 1991, all bronze, with the most recent coming in 2009 in Rome. They did not make the final at their two most recent appearances at the tournament, finishing ranked 11th in 2015 and ninth in 2019.
