Canada have won five Olympic medals in the men's 4x100m medley relay, including back-to-back silvers in 1984 and 1988. All five of their medals at the Olympic Games came in a 20-year period between 1972 and 1992. The only Games during those two decades where they did not win a medal was the 1980 edition in Moscow, which Canada boycotted.

When the Canadians last stood on an Olympic podium, in 1992 in Barcelona, they claimed bronze behind the United States of America and the Unified Team. At Rio 2016, the Canadian team recorded the slowest time in the heats and placed 16th from 16 teams.

Canada and the United States of America are the only two teams to have swam in the event at all 18 editions of the world championships. However, Canada have only managed one world championship medal, a bronze at the inaugural edition in 1973. At the 2019 World Championships in Gwangju, they failed to progress to the final and ranked 10th. They have not advanced to the world championship final in four attempts since 2011.

The Canadian team have won a medal at 13 of the 14 Pan American Games at which they have competed, including silver at six consecutive Games between 1967 and 1987.
