The United States of America have won a medal at each edition of the Paralympic Games in which wheelchair rugby has been part of the programme. They arrive in Tokyo as the reigning Paralympic silver medallists and their tally of six medals is the most of any nation. They have also won three gold medals, including at the inaugural edition in 1996 when wheelchair rugby was featured as a demonstration sport. The US team qualified for Tokyo by winning the 2019 Parapan American Games title.

The United States of America defeated hosts Australia to claim gold at the 2000 Paralympic Games. In Athens four years later, they overcame Great Britain in the bronze medal match. At Beijing 2008, the US team secured another gold with victory over Australia. The 2012 Games in London saw them edged out 50-49 by Canada in the semifinals before they bounced back to beat Japan to bronze. It was heartbreak for the US team in Rio in 2016, where they lost by just one point to Australia in the gold medal match, 59-58.

They have a similarly impressive record at the world championships, and have won seven medals [four gold] in seven appearances since 1995. However, they have had to settle for bronze at the last two tournaments in 2014 and 2018. At the 2018 World Championship in Sydney, the US team were beaten by eventual champions Japan in the semifinals before recovering to defeat Great Britain to finish third.
