The Islamic Republic of Iran secured a spot at Tokyo 2020 by reaching the semifinals at the 2019 Asia Oceania Championships in Thailand.

The Iranian team have competed at three editions of the Paralympic Games, participating in 2004, 2008 and 2016. At Rio 2016 they missed out on the knockout rounds after placing fifth in Group B, following a record of two wins and three losses. They then lost their classification match against Japan to finish 10th. The Iranians have only advanced from the group stage on one occasion, reaching the quarterfinals in Beijing in 2008. At the 2008 Games, they lost their quarterfinal against the United States of America on forfeit [20-0], as well as their subsequent classification games, after withdrawing from the tournament due to dissatisfaction with the match draw and schedule. At the 2004 Games in Athens, they beat Brazil in the ninth-place play-off match after failing to progress to the knockout rounds.

The Islamic Republic of Iran made their debut appearance at the world championships in 2014, where they advanced through two group phases before being eliminated by the United States of America in the quarterfinals. At the 2018 edition, they overcame the Republic of Korea and Turkey in the knockout rounds to face Great Britain for a place in the final. They lost to the British team 86-63 in the semifinal and were defeated 68-57 by Australia in the bronze medal match.
