The United States of America qualified for Tokyo after finishing second at the 2019 Parapan American Games in Lima, Peru. The US team lost the final to Brazil in Lima, but were awarded the qualification berth on offer as Brazil had already qualified for Tokyo.

The US team have a long Paralympic tradition in the sport. They took part in the first Paralympic goalball tournament in 1976, won their first and only Paralympic gold on home soil in 1984, and competed at every edition until they failed to qualify for the 2012 Games in London. The US team returned to the Paralympic stage in 2016, recording victories over Germany and Brazil in the knockout rounds before falling to Lithuania in the final. They also lost the final of the 1988 Games to Yugoslavia and finished second behind West Germany at the 1980 Games. The US team's five Paralympic medals are the most medals of any colour won by any nation in men's goalball.

At the world championships the US team have not fared as well, claiming two bronze medals in 2006 and 2014. At the 2018 edition, the US team were eliminated in the quarterfinals by Lithuania, who they beat in the third-place match at the 2014 edition.
