The ROC 4x100m mixed medley relay team secured a spot at Tokyo 2020 through a fourth-place finish at the 2019 World Championships in Gwangju. The Russian Federation have competed at all three editions of the world championships to feature the event, but are yet to win a medal. They set a national record at the 2019 edition of the tournament, and were only 0.1s behind Great Britain in the bronze medal position.

At the European championships, the Russian team won silver in 2018 and bronze in 2014. They placed fourth at the 2020 European Championships, which took place in May 2021. The Russians were gold medallists in the event at the 2015 European Games in Baku.
