The United States of America have won five out of the six Olympic gold medals on offer since the women's 4x200m freestyle relay was first introduced at the 1996 Games in Atlanta. In their five victories, the closest any team has come to beating them was the Australian team that finished 0.72s behind the US at the 2000 Games in Sydney. The US team's biggest Olympic final winning margin was the 2.55s gap ahead of the Chinese team that finished second at the 2004 Games in Athens.

At the 2016 Games in Rio the US team finished first ahead of Australia, just as they did four years earlier in London, but their time in Rio was actually slower than their winning time at the 2012 Games.

The only 'blemish' on the US team's Olympic record is a bronze medal at the 2008 Games in Beijing, where the top four all finished under the previous world record time, with the US finishing just over two seconds behind the victorious Australian team and 0.4s behind the second-place Chinese team.

At the world championships, the US team have won seven out of nine titles since 2003. In 2009, they were denied victory by the People's Republic of China who produced a world-record swim to win by 0.48s. At the most recent edition in 2019, Australia broke the 10-year-old Chinese record to beat the US team by 0.37s, with both teams swimming under the previous world record time.
