Denmark are making their sixth Olympic appearance in the women's 4x100m medley relay in Tokyo. At the 2016 Games in Rio de Janeiro they qualified third-fastest from the heats and matched that performance in the final, claiming bronze just 0.01s behind runners-up Australia to take home their first Olympic medal.

Denmark's Olympic participation in the event started with a disqualification on their first appearance at the 1988 Games in Seoul. It was not until their fourth appearance in 2012 that they made their first final, finishing seventh in London.

At the world championships the Danish women qualified for their first world final in 2015, and finished fifth. They did not progress to the final at the most recent edition in 2019, however, finishing ranked 16th after the heats.
