Hong Kong, China are making their third Olympic appearance in the event at Tokyo 2020. At their Olympic debut at Los Angeles 1984, they swam the 12th-fastest time in the heats, and did not make the final. In Seoul four years later, they were the 14th-fastest team in the heats and again missed out on the final.

At the 2019 World Championships in Gwangju, Republic of Korea, they swam the 10th-fastest time in the heats. It was not enough to qualify for the final, but it was Hong Kong, China's best time and rank in five total appearances at the world championships.

Hong Kong, China won bronze at the 2018 Asian Games in Indonesia behind the People's Republic of China and gold medallists Japan. It was their fourth consecutive bronze in the event at the Games.
