Since declaring independence from the Soviet Union in 1990, Lithuania have contested the men's 4x100m medley relay at the Olympic Games twice. They did not reach the final on either occasion, placing 18th in 1996 and 14th from 16 teams at the 2016 Games in Rio.

Lithuania have competed in the event at every edition of the world championships since 2003, but are yet to progress from the heats. At the most recent edition in 2019, they achieved a highest-ever ranking of 11th.

Their best result at continental level has come at the 2018 European Championships, when they just fell short of the podium with a fourth-place finish in Glasgow, Scotland. At the European championships in 2021 they narrowly missed out on the final with the ninth-fastest time in the heats.
