Germany have qualified for the final of the men's 4x100m medley relay at every Olympic Games they have contested since reunification. In that time they won bronze in 2000 and silver in 2004. Prior to reunification, both the German Democratic Republic and the Federal Republic of Germany won Olympic medals in the event, as did the Unified Team of Germany that took silver at the 1964 Games in Tokyo.

After failing to qualify for Beijing 2008, Germany ranked sixth at the 2012 Games, which was the first time they had finished outside the top four in five Olympic appearances since 1992. In Rio in 2016, they ranked one place lower in seventh.

At the world championships the German team have advanced to the final at five of the last six editions, winning silver in 2009 and bronze two years later. They missed the final in 2017, but returned to the elite race in 2019 in Gwangju, where they finished eighth.

They also ranked eighth at the European championships in 2021, having claimed bronze at the 2018 edition in Glasgow, Scotland.
