Since the women's 4x100m relay first appeared at the 1928 Games, no other team has featured at as many Games or won as many medals in the event as the United States of America. The US team have contested the event at 19 Olympic Games and won 15 medals, including 11 golds. Their nearest rival for total medals is Great Britain on eight, and for gold medals it is the German Democratic Republic on two.

A notable run during the US team's dominance in the event is the four consecutive gold medals they won between 1984 and 1996. But after winning bronze at Sydney 2000, the US team botched a baton exchange in the final at the 2004 Games, and four years later a dropped baton in Beijing meant they didn't even progress from the heats at the 2008 Games. However, they would redeem themselves in style at the 2012 Games as they smashed the then-world record by 0.55s, winning gold in London in 40.82s and more than half a second ahead of second-placed Jamaica. Four years later in Rio, they won gold again ahead of Jamaica, but were not quite as fast as their 2012 winning time.

At the world championships the US team have won more gold medals than any other nation with seven, two more than Jamaica, but the Jamaican team boasts more total medals with 15 compared to the US team's 12 medals. The two nations often battle for supremacy at world-level events, and between them have shared equally the last eight world championship titles. The US team claimed victory in 2005, 2007, 2011 and 2017, while the Jamaican team triumphed in 2009, 2013, 2015 as well as at the most recent edition in 2019 where the US team had to settle for bronze.
