Italy took part in the first Olympic women's 4x100m relay at the 1928 Games in Amsterdam, where they placed sixth. Since then they have contested another five Games, recording a best result of fourth in 1936. They did not qualify for the two most recent editions of the Games, while at Beijing 2008 they were disqualified in the heats in what was their first Olympic appearance for 20 years.
They secured their ticket to Tokyo at the 2019 World Championships in Doha, where they finished last out of the seven teams that posted an official result in the final. It was their equal-best showing at the world championships, matching their performance at the 1991 edition. Had they not qualified for Tokyo via the world championships, they still would have earned a berth through their performance at the 2021 World Relays, where they won gold.
