The United States of America won silver at the 2019 World Championships in Gwangju, finishing just 0.02 seconds behind gold medallists Australia in a thrilling final, as Australia's Cate Campbell edged out Simone Manuel of the US team on the anchor leg.

At the 2017 World Championships, the United States of America won the title while Australia took silver. When the event was introduced to the world championships at the 2015 edition, the US team claimed silver behind Great Britain.

The United States of America were disqualified from the final at the 2019 Pan American Games in Lima, Peru. They had finished in first place, but the disqualification was announced after the race due to an illegal leg movement by their breaststroke swimmer.
