Australia arrive in Tokyo as reigning world champions in the event following their triumph at the 2019 World Championships in Gwangju. In the final they edged out the United States of America by just 0.02 seconds after Australia's Cate Campbell chased down US swimmer Simone Manuel on the anchor leg.

Their victory in 2019 was a measure of revenge for the Australian team as they had finished in the silver medal position at the 2017 World Championships, almost three seconds behind their US rivals.
