Italy booked their ticket to Tokyo by recording one of the four fastest times in the qualification period among teams that did not qualify at the 2019 World Championships. The 2020 Olympic Games will be their sixth consecutive Olympic appearance, since making their debut in the event in 2000.

The Italian team have yet to win an Olympic medal, coming closest at the 2008 Games in Beijing, where they placed fourth behind medallists the United States of America, the People's Republic of China and Australia. They arrived at the 2016 Games as reigning world championship silver medallists, but did not qualify for the final in Rio de Janeiro, swimming the 13th-fastest time in the heats.

Their second-place finish at the 2015 World Championships in Kazan is their best performance in 11 world championship appearances since 1986. They failed to reach the final at the 2017 edition, and did not compete in 2019 in Gwangju.

Italy arrive in Tokyo having finished third at the European championships in May 2021. It was their first medal at the tournament since they won back-to-back titles in 2012 and 2014.
