The People's Republic of China are making their 10th Olympic appearance in the women's 4x100m medley, having debuted at the 1984 Games in Los Angeles. They have claimed two bronze medals since then, first in 1996 in Atlanta and again in front of home fans at the 2008 Games in Beijing. They have finished fourth on three occasions, beginning at the 1992 Games in Barcelona, which was the first time they reached an Olympic final, and again in 2004 and 2016. In Rio de Janeiro they finished 0.17s behind bronze medallists Denmark.

At the world championships the Chinese team have claimed four world titles, as well as one silver medal and two bronze medals. They set a then-world record to win a surprise gold at the 1994 World Championships, and claimed gold again in 2003, 2009, and 2015. From 1994 until their most recent world title in 2015, the Chinese team never finished outside the top four. However, their performances have dipped in recent years with the team placing sixth in 2017 and fifth at the 2019 edition in Gwangju.
