Canada have competed in all but one of the 15 Olympic women's 4x100m medley relays, only missing the 1980 Games in Moscow due to a boycott that came in the middle of their most successful period in the event. They claimed three bronze medals in a 12-year span that started in Montreal at the 1976 Games and ended at the 1988 Games in Seoul.

In recent years, Canada posted their worst Olympic performance in the event at the 2012 Games in London where they failed to make the final and finished ranked 12th overall. They bounced back four years later in Rio de Janeiro, however, qualifying for the final and finishing fifth in a then-national record time.

In 2019 Canada claimed their first medal at the world championships in the event, finishing third in Gwangju in a new national record time that was 0.16s behind runners-up Australia. It was Canada's first world championship podium in the event in 18 appearances at the tournament.
