Australia and Great Britain are the only two nations to have competed in the women's 4x100m medley relay at every Olympic Games since the event was introduced in 1960. In 2004 Australia became the third team, after the United States of America and the German Democratic Republic, to win gold in the event. They finished almost two seconds ahead of the US team in a then-world record time of 3:57.32.

At Beijing 2008, the Australians retained their Olympic title with another world record at the time, and again ahead of the US team. The trend was reversed in 2012 and 2016 however, with the US team forcing Australia into the silver medal position in both London and Rio de Janeiro.

The team's silver medal in Rio de Janeiro was the sixth consecutive Games in which Australia had finished with either silver or gold in the event, and they sit second behind the US in terms of total medals won with eight Olympic podium finishes compared to the US team's 14.

Australia's world championship record in the event is similarly impressive, with an Australian team making the podium at every edition of the tournament since 1998.
