The United States of America share the record with Great Britain for 22 Olympic appearances in the women's 4x100m freestyle relay, and have finished on the podium at each Games they have entered the event.

The only editions of the event the US have missed were the inaugural staging of the event at the 1912 Games in London, and the boycotted 1980 Games in Moscow. Their 22 Olympic medals stands head and shoulders above their nearest rivals, the Netherlands, who boast 12 total medals. Of the 22 medals, the US have won 14 gold, which is also well clear of second-placed Australia on four Olympic golds.

However, despite their historic dominance in the event, the US team have not won gold at the Games since Sydney 2000. They claimed silver in Rio in 2016 behind Australia, bronze behind the Netherlands and Australia in 2012, silver behind the Netherlands in 2008, and silver behind Australia in 2004.

It is a similar story at the world championships, where the US team have only failed to win a medal once in 18 attempts, with their 17 total medals almost double their nearest rivals Australia and the Netherlands, both on nine. However, their dominance on the top step of the podium is less pronounced, with six golds for the US compared to Australia's four. At the most recent edition in 2019, the US team set a new Americas record but it was not enough to finish ahead of their Australian rivals and they had to settle for second place.
