The ROC 4x100m medley relay team qualified for the 2020 Olympic Games through a third-place finish at the 2019 World Championships in Gwangju.

Previously, the Russian Federation competed in the event at six consecutive Olympic Games between 1996 and 2016. Their only medal during this period was a silver on debut in 1996, although they have come close to the podium three times since, ranking fourth in 2004, 2008 and 2016. Russian swimmers competed at the Games as part of the Soviet Union between 1960 and 1988, winning one silver and two bronze medals.

On the world championship stage, the Russian Federation's seven total medals is bettered only by the United States of America's 15, with Australia also having won seven medals. The Russian Federation claimed consecutive third-place finishes at the last two editions in 2017 and 2019, behind the US team and Great Britain on both occasions. They have yet to win world championship gold.

The Russian Federation's tally of 11 total European championship medals is the most of any nation, and they have stood on the top step of the podium on seven occasions. At the European championships in both 2021 and 2018, they placed second behind Great Britain.
