General
At London 2012, Japan achieved their best Olympic result in the 4x100m medley relay by finishing second behind the United States of America. It was Japan's third consecutive Olympic medal in the event after they won bronze at both the 2004 and 2008 Games. They also claimed the bronze medal when the event made its Olympic debut in 1960 in Rome.

Japan have competed at every Olympic Games to feature the event except the 1980 Games, which they boycotted, and the 2000 Games in Sydney. They have qualified for the final at every appearance, and ranked fifth at Rio 2016.

Like at the Olympic Games, the Japanese team have collected four medals at the world championships but have never won gold. They most recently made the podium in 2013 in Barcelona, where they took bronze, with their best result a silver medal at the 2007 edition. They have come close to a medal at the last two world championships, finishing fourth in both 2017 and 2019.

Japan are the dominant nation at the Asian Games, amassing 16 medals from 16 participations, including 13 golds. However, they have had to settle for silver at the last two Asian Games, behind the People's Republic of China on both occasions.
