Tokyo 2020 will be the People's Republic of China's eighth Olympic appearance in the men's 4x100m medley relay, with the team yet to win a medal at the Games. They made their debut at the 1984 Games in Los Angeles, and competed at the last two Games in 2012 and 2016 following a 12-year absence.

They qualified for the final at the 2016 Games after recording the equal-fourth fastest time in the heats. In the final they were disqualified from the race because of an early relay takeover on the butterfly leg.

The Chinese team are also yet to claim a medal at the world championships. However, at the two most recent editions they have progressed to the final having previously never reached the elite race in eight attempts. They went on to finish sixth in 2017, and seventh in 2019.

The People's Republic of China have enjoyed more success at continental level. Their total medal tally of 11 at the Asian Games is second only to Japan's 16, and the team won back-to-back gold medals in 2014 and 2018.
