Brazil are making their 13th Olympic Games appearance in the 4x100m medley relay, and have swam in the final at three Games. Their best result has been a fifth-place finish at Munich 1972. At the 2016 Games in Rio de Janeiro, the home team ended a run of six Olympic appearances without reaching the final when they advanced to the elite race and ranked sixth.

Brazil have made the final at the last two world championships, having failed to progress past the heats at the previous three editions. They were fifth and 2017 and sixth in 2019, with the team's best world championship result a fourth-place finish in 2009.

The Brazilian team have won a medal at each edition of the Pan American Games in which they have competed. They have finished on the top step of the podium three times, including consecutive gold medals in 2011 and 2015, ahead of the United States of America on both occasions. At the most recent edition in 2019, the US team turned the tables to win gold, with Brazil having to settle for silver.
