Only the United States of America, with 14, have won more Olympic medals in the men's 4x100m medley relay than Australia's total of nine. Indeed, Australia are the only other Olympic gold medallists in the event after taking the title at the 1980 Games in Moscow, where the United States of America were part of a group of boycotting NOCs.

Australia won silver in the inaugural Olympic 4x100m medley relay in 1960 and along with Great Britain, are one of two teams to have competed at every Games to include the event. In addition to their one gold in 1980, the Australian team have claimed three silver and five bronze medals.

Since the 1996 Games in Atlanta, Australia have only missed the podium on one occasion, in 2004, when they finished third in their heat behind Ukraine and the Russian Federation and did not progress to the final. They won successive bronze medals at the Olympic Games in 2012 and 2016, missing out on silver behind Japan and Great Britain, respectively.

Australia have been world champions in the event on three occasions, twice in home water in 1998 and 2007, and also at the 2001 edition in Fukuoka, Japan. After claiming a medal at each edition of the world championships between 2007 and 2015, they have failed to reach the podium in their last two attempts. In 2017 they were unable to advance from the heats, and in 2019 they ranked fifth.
