The United States of America have won a medal at every one of the 12 Olympic Games to feature the men's 4x100m freestyle relay event, and nine of those medals have been gold. They claimed consecutive gold medals at the seven editions of the Games to feature the event from 1964 to 1996. However, the 21st century has seen the US team's dominance of the event decrease, as Australia [2000], South Africa [2004], and France [2012] claimed gold at three out of the five Games since 2000. The US team secured gold at the 2008 Games in Beijing, and are the reigning Olympic champions having reclaimed their title by beating France to gold at the 2016 Games in Rio de Janeiro.

At the world championships the US team have won 16 medals, including 13 gold. Their nearest rivals on the medal table are Australia and France on two golds apiece. The US team failed to qualify for the final in 2015, but rebounded from that disappointment to claim back-to-back world titles in 2017 and 2019. At the 2019 edition in Gwangju, their time of 3:09.06 was a world championship record but was not as fast as the world record time of 3:08.24 they recorded in the final at the 2008 Olympic Games in Beijing.
