No other men's 4x400m relay team have won as many Olympic or world championship gold medals as the United States of America. In their 22 Olympic appearances in the event - a record bettered only by Great Britain's 23 appearances - they have won a total of 20 medals, including 17 gold. Their nearest rival for total Olympic medals is the 12 won by Great Britain.

The US team are the reigning Olympic champions, having claimed gold for the third time this century at the 2016 Games in Rio. Four years earlier, the Bahamas beat the US team to gold in London by 0.33s. The US team were triumphant in Beijing in 2008, which remains their fastest Olympic-winning time to date, as well as at Athens in 2004. Their gold medal from the Sydney 2000 Games was disqualified retrospectively.

At the world championships the US team won six titles in a row from 2005 to 2015 before their run was ended by Trinidad and Tobago at the 2017 edition of the tournament. The US team resumed normal service at the 2019 edition, finishing ahead of Jamaica and Belgium to claim their seventh title from the last eight world championships, and a 10th gold overall.

The US team also won gold at the first three editions of the World Relays.
