Laila Mohsen and Hanna Hiekal were assured of a berth in the duet competition in Tokyo courtesy of Egypt's performance in the team event at the 2019 World Championships in Gwangju, where they finished as the highest-ranked African nation.

Tokyo will be the Olympic debut of both artistic swimmers. At the 2019 World Championships in Gwangju, Mohsen competed alongside Dara Tamer in the technical routine, while Hiekal partnered Jayda Sharaf in the free routine, achieving 28th and 27th-place rankings, respectively.
